Habilitator Openings Residential Community Day Program OPTS
Provides supervision, training, and assistance in accomplishing activities of daily living in IRA (Individual Residential Alternative) which includes, but is not limited to, self-care skills, social skills, recreation skills, community skills, and domestic skills. Assists consumers in providing for their dietary needs including, but not limited to, meal preparation, meal planning, grocery shopping, etc. Initiates structured and unstructured recreational and leisure-time activities consistent with the consumer’s goal plan. Transports consumers as directed. Will become medication certified and will pass meds as required. Employee must be able to train in and perform Strategies for Crisis Intervention and Prevention - Revised (SCIP-R) Must be able to lift, transfer, and move at least 50 pounds and perform CPR and first aid.
Home Health Aides/Personal Care Aides
Provides oversight, training and assistance to consumers in accomplishing activities of daily living in their home. Including, but not limited to, personal care, self-care skills, recreation skills, community skills, and domestic skills. Assists consumer in providing for his or her dietary needs including, but not limited to, meal preparation, meal planning, grocery shopping, etc. Initiates structured and unstructured recreational and leisure time activities consistent with the consumer’s goal plan. Transports consumer as needed. Must have the ability to lift and transfer 50 pounds.
