PRALID Home Services: PRI/Screen
The Patient Review Instrument/Screen (PRI/Screen) is an evaluation tool performed by an RN that determines eligibility for adult and nursing home services. At PRALID we also use it to determine eligibility for the Traumatic Brain Injury and Nursing Home Transition/Diversion Waivers which we are an approved provider.
